    This page is designed to help you register as a new patient, request a new appointment, understand what insurance we accept, and learn what you need to bring with you to your visit.     New Patient Registration

    If you're new to West Metro Ophthalmology, you'll need to register as a patient before scheduling your first appointment. We use a secure service called DemandForce for new patient registration, which keeps your personal health information safe and protected.

    While you can complete your registration in person at the clinic, we strongly encourage you to fill out the online registration form before your visit if possible. This helps us prepare for your appointment and reduces your wait time when you arrive.

    Insurance Coverage

    West Metro Eye is contracted with many of the major medical insurance carriers in our region. Because our doctors are classified as specialists, your insurance policy may require certain prerequisites—such as referrals, prior authorizations, or other restrictions—before you can be seen. These requirements can affect claim reimbursement, and specialist co-pays may apply for which you are responsible.

    Important Note on Vision Plans

    West Metro Eye does not participate in vision carve-out plans (also called standalone vision benefits), such as VSP or EyeMed. These plans, often provided through employers or insurers, cover only routine eye care and refractions (wellness exams) and do not cover medical eye conditions or treatments.

    If you have a vision carve-out plan, we are happy to continue caring for you. However, we will bill your medical insurance for the visit when monitoring, diagnosing, or treating medical eye conditions, including (but not limited to):

    • Cataracts
    • Glaucoma
    • Diabetic eye changes
    • Macular degeneration
    • Corneal diseases
    • Dry eye

    For questions about your coverage—including whether West Metro Eye is in-network with your specific medical plan, whether your medical plan uses a vision carve-out for routine care, or any co-pays you may owe—please:

    • Check your benefits online through your insurance portal, or

    What to Bring With You to Each Visit

    When you arrive for your first appointment, please bring the following:

    • Photo ID (driver's license or state ID)
    • Medical insurance card
    • List of current medications
    • Current eyeglasses or contact lenses or a paper copy of your most recent glasses prescription
    • Your Primary Care Provider's information
    • Referring Provider information (if applicable)
    • Referral paperwork and previous eye exam records (if available/required and not already sent by your referring provider)
    • If the patient is under 18, a parent or legal guardian must be present at the visit, or a signed Authorization to Evaluate & Treat a Minor form must be provided (available on our Patient Forms page)
    • Any required/needed forms (if applicable)

    Please arrive 10-15 minutes before your scheduled appointment to complete any remaining paperwork. If you need to cancel or reschedule, please call us at least 24 hours in advance at (763) 546-8422.
